BLR 5708 - Proposal Bid Bond (if required) By Order of _ United City of Yorkville (Awardino Authority) Deborah Simmons County Encineer/County Superintendent of Hichways/Municioal clerk Note: All proposal documents, including Proposal Guaranty Checks or Proposal Bid Bonds, should be stapled together to prevent loss when bids are processed. c� SPECIAL PROVISIONS The following Special Provisions supplement the "Standard Specifications for Road and Bridge Construction", adopted January 1, 1997, (hereinafter referred to as the Standard Specifications); the latest edition of the "Manual on Uniform Traffic Control Devices for Streets and Highways" in effect on the date of invitation for bids; the"Supplemental Specifications and Recurring Special Provisions", dated January 1, 2000 and indicated on the Check Sheet included herein, which apply to and govern the proposed improvement designated as COUNTRYSIDE PARKWAY ROADWAY IMPROVEMENTS, SECTION NO.99-00021-00-FP", and in case of conflict with any part or parts of said specification, standards, or criteria,these Special Provisions shall take precedence and govern. DESCRIPTION OF WORK: Work consists primarily of bituminous pavement removal and replacement on Countryside Parkway for a distance of 598 feet (0.1132 miles). Also included is aggregate base repair as necessary, removal of gutter overlays, thermoplastic pavement marking, and traffic control. PREQUALIFICATION OF BIDDERS: Prequalification of bidders in accordance with Section 102.01(b) of the Standard Specifications will be required of all bidders on this project. BID SECURITY: Bid security shall be in the form of a bid bond, bank draft, certified or cashier's check in the amount of five(5%) percent of the bid proposal. MATERIAL TICKETS: The Contractor shall furnish the Engineer with material delivery tickets for all materials furnished in conjunction with the construction of the project including, but not limited to, aggregate, all bituminous mixes, concrete, conduit, and cable. The delivery tickets shall be given to the Engineer each day at the end of the working day. MATERIAL CERTIFICATION: The Contractor shall furnish certifications to the Engineer from all suppliers for all materials except those to be inspected by the Engineer. The statements shall certify that the materials comply with the requirements of the Standard Specifications and these Special Provisions. No payment will be made for any item or materials until proper certification is submitted to the Engineer. PERIODIC CLEANUP: The contractor shall perform general cleanup of the worksite at least once a week. No driveway shall be left closed overnight. This work shall not be paid for separately, but shall be considered incidental to the contract. BASELINES: All work shall be constructed in accordance with the lines as shown on the drawings or as given by the Engineer. Responsibility for keeping alignment shall belong to the Contractor. Lines shall be set by the Engineer as construction progresses. The Contractor shall use line references, as established by the Engineer, to establish his own working or construction SP-1 ;i lines as required at his own expense and shall be solely responsible for the accuracy thereof. The Contractor's work shall be subject to check and review of the Engineer. The Contractor shall safeguard all lines from damage by his operations, and shall replace at his expense such damaged markers as determined by the Engineer. The Contractor shall safeguard all property corners, monuments, and marks adjacent to but not related to the work, and if required shall bear the cost of restoring these if damaged or moved during construction. TRAFFIC CONTROL AND PROTECTION: Traffic control shall be in accordance with the plans, applicable sections of the Standard Specifications for Road and Bridge Construction, the applicable guidelines contained in the Illinois Manual on Uniform Traffic Control Devices for Streets and Highways, these special provisions, and any special details and Highway Standards herein and in the plans and the Standard Specifications for Traffic Control Items. The roadway shall be closed to through traffic during construction and a detour route signed in accordance with the details as shown on Plan Sheet 4. Special attention is called to the following sections of the Standard Specifications, the Highway Standards, and the special provisions relating to traffic control: Standard Specifications: Section 701 —Work Zone Traffic Control Section 702—Work Zone Traffic Control Devices Section 703 —Work Zone Pavement Markings Section 783 —Pavement Marking and Marker Removal Highway Standards: 701501 702001 701601 In addition, the following also relate to traffic control for this project: SPECIAL PROVISIONS: Traffic Control Deficiency Deduction SUPPLEMENTAL SPECIFICATIONS: 701 Work Zone Traffic Control 703 Work Zone Pavement Marking All applicable traffic control standards required and detour signage as shown on the Traffic Control Plan on Sheet 4 will be paid for as Traffic Control and Protection on a lump sum basis. AGGREGATE BASE REPAIR: This item consists of excavating unsuitable base course and subgrade areas to the depth indicated by the Engineer, and placing and compacting granular base course, Type B. Material gradation will be CA-6 unless directed otherwise by the engineer. This work shall be paid for at the contract unit price per TON, including excavation and disposal of unsuitable base, placement of new aggregate, and compaction and proof-rolling of the new aggregate. �, SP-2 N. GEOTECEMCAL FABRIC FOR GROUND STABILIZTION: In addition to the requirements �.. of Section 210 of the standard specifications, the contractor shall overlap the sides of any undercut excavation at least 12 inches. The geotechnical fabric shall be Amoco 4551 or approved equal. Geotechnical fabric shall be paid for at the contract unit price per SQUARE YARD. PREPARATION OF BASE: This item consists of shaping, compacting, proof-rolling and string- lining the remaining aggregate base course to the dimensions indicated on the typical section prior to paving binder course. Proof-rolling shall be made with a tandem-axle truck with a net weight of at least 14 tons as many rimes as necessary until a satisfactory test is witnessed. This item shall be paid for on a square yard basis. BUTT JOINT: This item consists of cutting a smooth vertical surface through existing bituminous pavement to allow for removal. Cutting shall be performed with a saw blade capable of cutting through the entire thickness of the item being cut. This item shall be performed at all locations where the proposed pavement meets existing pavement, or at locations designated in the field by the Engineer. This item shall also include a temporary ramp made of aggregate, millings, or asphalt for construction and local traffic to use while the roadway is under construction. This temporary ramp is not to be confused with temporary aggregate ramps at private driveways, which shall be paid for separately. Payment for BUTT JOINT shall be paid for at the contract unit price per foot of sawcut, for each location where a continuous butt joint is required. BIT. DRIVE REM&REPL: This item consists of removing existing bituminous driveway entrance, grading and compacting the existing aggregate base, and paving 4" of Bituminous Surface Course(2 lifts) at locations directed by the engineer. Payment shall be made on a square yard basis, complete and in place. Payment for saw cutting butt joints shall be made separately. TEMPORARY AGGREGATE FOR DRIVEWAYS: This item consists of supplying, placing, maintaining, removing, and disposing of aggregate ramps at private driveways. Aggregate to be CA-6 gradation. Ramp size and height shall vary depending on driveway size. This item shall be paid for on a tonnage basis. CRACK FILLING: This item consists of sealing the joint at the edge of pavement or other locations designated by the engineer. The work shall conform to Section 451 of the Standard Specifications. Payment shall be made on a linear foot basis. GUTTER OVERLAY REMOVAL—TYPE 1 and TYPE 2: These items consist of removing the existing bituminous overlay on the PCC gutter in accordance with the details in the plans and as directed by the engineer. For TYPE 2 removal, the contractor shall also edge grind the existing pavement to provide a smooth tapered edge down towards the gutter as indicated on the plans. Care shall be taken to avoid damaging the existing traffic signal detector loop cables. Payment shall be made on a linear foot basis, as measured along the flow line of the gutter. SPECIAL PROVISIONS The following Special Provisions supplement the "Standard Specifications for Road and Bridge Construction", adopted January 1, 1997, (hereinafter referred to as the Standard Specifications); the latest edition of the "Manual on Uniform Traffic Control Devices for Streets and Highways" in effect on the date of invitation for bids; the"Supplemental Specifications and Recurring Special Provisions", dated January 1, 2000 and indicated on the Check Sheet included herein, which apply to and govern the proposed improvement designated as COUNTRYSIDE PARKWAY ROADWAY IMPROVEMENTS, SECTION NO.99-00021-00-FP", and in case of conflict with any part or parts of said specification, standards, or criteria, these Special Provisions shall take precedence and govern. DESCRIPTION OF WORK: Work consists primarily of bituminous pavement removal and replacement on Countryside Parkway for a distance of 598 feet (0.1132 miles). Also included is aggregate base repair as necessary, removal of gutter overlays, thermoplastic pavement marking, and traffic control. PREQUALIFICATION OF BIDDERS: Prequalification of bidders in accordance with Section 102.01(b) of the Standard Specifications will be required of all bidders on this project. BID SECURITY: Bid security shall be in the form of a bid bond, bank draft, certified or cashier's check in the amount of five (5%) percent of the bid proposal. MATERIAL TICKETS: The Contractor shall furnish the Engineer with material delivery tickets for all materials furnished in conjunction with the construction of the project including, but not limited to, aggregate, all bituminous mixes, concrete, conduit, and cable. The delivery tickets shall be given to the Engineer each day at the end of the working day. MATERIAL CERTIFICATION: The Contractor shall furnish certifications to the Engineer from all suppliers for all materials except those to be inspected by the Engineer. The statements shall certify that the materials comply with the requirements of the Standard Specifications and these Special Provisions. No payment will be made for any item or materials until proper certification is submitted to the Engineer. PERIODIC CLEANUP: The contractor shall perform general cleanup of the worksite at least once a week. No driveway shall be left closed overnight. This work shall not be paid for separately, but shall be considered incidental to the contract. BASELINES: All work shall be constructed in accordance with the lines as shown on the drawings or as given by the Engineer. Responsibility for keeping alignment shall belong to the Contractor. Lines shall be set by the Engineer as construction progresses. The Contractor shall use line references, as established by the Engineer, to establish his own working or construction SP-1 } lines as required at his own expense and shall be solely responsible for the accuracy thereof. The Contractor's work shall be subject to check and review of the Engineer. The Contractor shall safeguard all lines from damage by his operations, and shall replace at his expense such damaged markers as determined by the Engineer. The Contractor shall safeguard all property corners, monuments, and marks adjacent to but not related to the work, and if required shall bear the cost of restoring these if damaged or moved during construction. TRAFFIC CONTROL AND PROTECTION: Traffic control shall be in accordance with the plans, applicable sections of the Standard Specifications for Road and Bridge Construction, the applicable guidelines contained in the Illinois Manual on Uniform Traffic Control Devices for Streets and Highways, these special provisions, and any special details and Highway Standards herein and in the plans and the Standard Specifications for Traffic Control Items. The roadway shall be closed to through traffic during construction and a detour route signed in accordance with the details as shown on Plan Sheet 4. AGGREGATE BASE REPAIR: This item consists of excavating unsuitable base course and subgrade areas to the depth indicated by the Engineer, and placing and compacting granular base course, Type B. Material gradation will be CA-6 unless directed otherwise by the engineer. This work shall be paid for at the contract unit price per TON, including excavation and disposal of unsuitable base, placement of new aggregate, and compaction and proof-rolling of the new aggregate. �.. SP-2 GEOTECUMCAL FABRIC FOR GROUND STABILIZTION: In addition to the requirements `—' of Section 210 of the standard specifications, the contractor shall overlap the sides of any undercut excavation at least 12 inches. The geotechnical fabric shall be Amoco 4551 or approved equal. Geotechnical fabric shall be paid for at the contract unit price per SQUARE YARD. PREPARATION OF BASE: This item consists of shaping, compacting, proof-rolling and string- lining the remaining aggregate base course to the dimensions indicated on the typical section prior to paving binder course. Proof-rolling shall be made with a tandem-axle truck with a net weight of at least 14 tons as many times as necessary until a satisfactory test is witnessed. This item shall be paid for on a square yard basis. BUTT JOINT: This item consists of cutting a smooth vertical surface through existing bituminous pavement to allow for removal. Cutting shall be performed with a saw blade capable of cutting through the entire thickness of the item being cut. This item shall be performed at all locations where the proposed pavement meets existing pavement, or at locations designated in the field by the Engineer. This item shall also include a temporary ramp made of aggregate, millings, or asphalt for construction and local traffic to use while the roadway is under construction. This temporary ramp is not to be confused with temporary aggregate ramps at private driveways, which shall be paid for separately. Payment for BUTT JOINT shall be paid for at the contract unit price per foot of sawcut, for each location where a continuous butt joint is required. BIT. DRIVE REM&REPL: This item consists of removing existing bituminous driveway entrance, grading and compacting the existing aggregate base, and paving 4" of Bituminous Surface Course(2 lifts) at locations directed by the engineer. Payment shall be made on a square yard basis, complete and in place. Payment for saw cutting butt joints shall be made separately. TEMPORARY AGGREGATE FOR DRIVEWAYS: This item consists of supplying, placing, maintaining, removing, and disposing of aggregate ramps at private driveways. Aggregate to be CA-6 gradation. Ramp size and height shall vary depending on driveway size. This item shall be paid for on a tonnage basis. CRACK FILLING: This item consists of sealing the joint at the edge of pavement or other locations designated by the engineer. The work shall conform to Section 451 of the Standard Specifications. Payment shall be made on a linear foot basis. GUTTER OVERLAY REMOVAL—TYPE 1 and TYPE 2: These items consist of removing the existing bituminous overlay on the PCC gutter in accordance with the details in the plans and as directed by the engineer. For TYPE 2 removal, the contractor shall also edge grind the existing pavement to provide a smooth tapered edge down towards the gutter as indicated on the plans.
