Resolution 1997-07 STATE OF ILLINOIS ) ss. COUNTY OF KENDALL ) RESOLUTION No: 97- RESOLUTION APPROVING THE EMPLOYEE ASSISTANCE PROGRAM WITH MERCY CENTER FOR HEALTH CARE SERVICES AND AUTHORIZING THE MAYOR OF THE UNITED CITY OF YORKVILLE AND THE CITY CLERK TO ENTER INTO THE AGREEMENT ON BEHALF OF THE CITY OF YORKVILLE WHEREAS, the Mayor and the City Council of the UNITED CITY OF YORKVILLE have reviewed the proposed Employee Assistance Program with MERCY CENTER FOR HEALTH CARE SERVICES; and WHEREAS, the Mayor and the City Council have determined that it is in the best interest of the citizens of the City of Yorkville, and the Employees of the City of Yorkville to enter into such agreement; and WHEREAS, the agreement with MERCY CENTER FOR HEALTH CARE SERVICES will provide needed assistance for the employees of the City of Yorkville will be beneficial to the City of Yorkville and its Employees. NOW THEREFORE BE IT RESOLVED by the UNITED CITY OF YORKVILLE: I. That the Employee Assistance Program with MERCY CENTER FOR HEALTH CARE SERVICES is hereby approved by the UNITED CITY OF YORKVILLE and the Yorkville City Council. 2. That the Mayor and the City Clerk of Yorkville are hereby authorized to enter into the Employee Assistance Program Agreement with MERCY CENTER FOR HEALTH CARE SERVICES on behalf of the City. Bridge Street Yorkville, Illinois 60560 630.553.9500 2 down -ininn MERCY CENTER for HEALTH CARE SERVICES EAP CONTRACT- LETTER OF AGREEMENT This serves as confirmation of Agreement regarding the contract between the City of Yorkville (Employer) and Mercy Center for Health Care Services, for Employee Assistance Program services. It is agreed that, as of May 1, 1997, the terms of the original Agreement will remain, except for the number of employees and the following addition: OWNERSHIP AND CONFIDENTIALITY OF EAP RECORDS: B. Unless Employer states otherwise, in writing, Mercy Center may use Employer's name for a reference. EXHIBIT B, 1. FEES: A. The fee for EAP services will be charged at $25.00 per full time employee, per year, per terms of this agreement. B. The current number of full time employees at the City of Yorkville is 41. This number will be used in billing, unless the City of Yorkville informs Mercy Center of an increase or decrease of 10% of the employee population. Renewal Options: X This agreement is effective for 12 months, from 5/1/97 through 4/30/98. This agreement is effective for 24 months, from 5/1/97 through 4/30/99.
