RESOLUTION NO. 97- 1
RESOLUTION APPROVING A CONTRACT WITH NYLCARE HEALTH PLANS OF
THE MIDWEST, INC. FOR EMPLOYEE HEALTH CARE
WHEREAS, NYLCARE has provided affordable Health Care and Life Insurance coverage for
the past year, and,
WHEREAS, NYLCARE has limited premium health care increases to 5% and life insurance
premium increases to 0%;
NOW THEREFORE BE IT RESOLVED that the Mayor and City Clerk are hereby authorized
to enter into contract with NYLCARE Health Plans of the Midwest, Inc. for said employee
health care and life insurance until April 30, 1998
