Resolution 2010-08 STATE OF ILLINOIS ) )ss COUNTY OF KENDALL ) RESOLUTION NO. 2010- p� RESOLUTION APPROVING A HEALTH INSURANCE OPT -OUT INCENTIVE POLICY WHEREAS, the City Council of the United City of Yorkville has considered and discussed establishing a policy concerning a health insurance opt -out incentive for city employees, and WHEREAS, the City Council has determined that it desires to establish such a policy, and WHEREAS, the text of the policy the City Council now desires to establish concerning a health insurance opt -out incentive is set forth on the attached Exhibit "A" which is incorporated herein, and NOW THEREFORE BE IT RESOLVED, by the Mayor and City Council of the United City of Yorkville, Kendall County, Illinois that the Employee Health Insurance Opt -Out Incentive Policy in the form set forth on Exhibit "A" attached hereto and incorporated herein is hereby approved. I MAYOR i I I I I� Exhibit A United City of Yorkville Employee Health Insurance Opt -Out Incentive Policy Introduction Per the City of Yorkville Employee Manual all full -time employees are eligible to be covered under the City's health insurance plan, which includes coverage for medical, dental, and vision benefits. While employees are required to pay a portion of the premium for their medical insurance, the City incurs most of the expense for the premium charged. Opt -Out Incentive Full -time employees who choose not to enroll in the City of Yorkville's medical health insurance plan are eligible to receive a monthly stipend. Employees previously enrolled in "single" coverage would be eligible to receive $100.00 per month. Employees previously enrolled in "employee plus spouse," "employee plus children," or "family" coverage would be eligible to receive $150.00 per month. Employees are required to provide proof of enrollment in another comparable health insurance plan (i.e. a spouse's plan) in order to receive the stipend. Exclusions Married couples who are both employees of the City of Yorkville are not eligible to receive the stipend, unless they can provide proof that both are enrolled in a comparable, non -City, health insurance plan.
